
Types of Evaluations
Comprehensive Adult Neuropsychology Evaluation

This includes any evaluation for age 18+ that is related to cognitive decline or a medical diagnosis. Neuropsychological evaluations for memory or cognitive changes are often covered by health insurance. If using health insurance, please note that a referral from a medical doctor (or nurse practitioner) is required to support medical necessity for insurance purposes. Just ask your doctor for a referral or script for a "neuropsychological evaluation" along with a diagnosis code, usually R41.89 (other symptoms and signs involving cognitive functions or awareness) or G31.84 (mild cognitive impairment, unspecified).
Comprehensive Pediatric Neuropsychology Evaluation

This includes any pediatric (age 12-17) evaluation that is related to a medical diagnosis (e.g., TBI/concussion, seizures, stroke, FAS, genetic conditions, etc.). These neuropsychological evaluations are often covered by health insurance. If using health insurance, please note that a referral from a medical doctor (or nurse practitioner) is required to support medical necessity for insurance purposes.
ADHD Testing

Ages 12-17: Brief ADHD-focused cognitive testing, interview with child and parent(s)/caregivers, and self- and informant-report measures will be administered.

Adults age 18-49: Brief ADHD-focused cognitive testing, interview with patient (significant other or family member are welcome as well), and self- and informant-report measures will be administered.

Adults age 50+: As a person ages, other neurological factors are more likely to influence your thinking. For example, I would not test only for ADHD in a 50+-year-old without also testing for broader cognitive decline. Therefore, those aged 50+ are asked to undergo a comprehensive neuropsychology evaluation (see first box on this page).
* At this time, ADHD evaluations are offered on a self-pay basis only. This is because most insurance plans consider ADHD testing not medically necessary and deny claims even after services are completed. Therefore, to avoid unexpected denials and financial disputes, we do not submit insurance claims for ADHD evaluations. If requested, Dr. Malinowski can provide information for you to submit an out-of-network claim to your insurance company for possible partial reimbursement.

Educational Evaluation

Educational evaluations are for ages 12-18. In addition to general cognitive ability, skills in reading, math, and/or writing are assessed. ADHD and psychological concerns are also screened and assessed more thoroughly, if indicated. Health insurances do not pay for educational testing, therefore, this type of evaluation is self-pay only.
Comprehensive Adult Baseline Testing

Comprehensive baseline testing is an in-depth neuropsychological evaluation. "Comprehensive" means that I will administer all tests that I would typically administer to a patient with suspected cognitive decline. This is a great option for those with a family history of dementia, those who want to closely monitor their brain health, or those who want insight into their cognitive strengths and weaknesses. With a baseline, if cognitive decline should occur in the future, we can compare scores to your baseline, apples to apples, to objectively quantify any areas of decline. Baseline testing is an out-of-pocket expense, as insurance does not consider is "medically necessary.."

*This option is only offered to patients without current cognitive concerns. If you have cognitive concerns, a comprehensive evaluation is more appropriate and likely to be covered by insurance.
Brief Adult Baseline Testing

If you want very brief baseline testing to which to compare future performance, this is a great option! This option is an out-of-pocket expense, as it is preventive in nature.
*This option is only offered to patients without current cognitive concerns. If you have cognitive concerns, a comprehensive evaluation is more appropriate and likely to be covered by insurance.
Pre-Surgical Evaluation

This includes pre-DBS, epilepsy, cardiac, or other major surgeries. This testing is slightly briefer than regular medical neuropsychological evaluations. These evaluations are typically covered by health insurance.
Adult IQ Testing (including for Mensa admission)

Whether you've always been curious about your IQ, or are looking to join Mensa, IQ testing with the Wechsler Adult Intelligence Scale (WAIS-5) is available! A brief report of scores, as well as the necessary single-page Mensa paperwork, will be produced. Please note that testing is not a guarantee of Mensa status, you may or may not get your desired results! IQ testing is also available for documenting an intellectual disability for the purpose of applying for services or benefits. IQ testing is an out-of-pocket service, not paid for by insurance.
